Dr. Tammy's Project Fabulous
At Northwest Health and Lifestyles Centre, we strive to fully understand not only our patient's main health concerns, but their complete health history. To achieve this, we schedule new patients for 45 minutes to one hour for their first visit. We ask that you arrive 5 to 10 minutes early to ensure that your visit is not rushed.
At the end of the first visit, we will discuss with you the appropriate testing, proposed plan of management, and related costs. Our hope is that we allow you to understand what is being recommended, why it is important, and what we plan to do with the test results we receive.
Insurance coverage varies widely, so please bring any insurance information to your first visit, and we will help you determine the role your health insurance may play.
We accept cash, check, Mastercard, and Visa. Special financing plans are available to those who qualify.
To download our New Patient Intake Forms in PDF format, please click the link to the left. Please print and fill out completely before your appointment. Additional forms may be required when you arrive.
